I am writing on behalf of a 78-year-old male client diagnosed with mild dementia who is currently living alone. During my visits with him, it was observed that he spends most of his time at home by himself, aside from occasionally going out to nearby coffee shops for his meals. As part of our efforts to encourage him to engage in more meaningful daily activities to help slow cognitive decline, we explored possible activities that he might enjoy. During our conversation, he shared that he used to play the accordion in the past and expressed how meaningful it would be if he could have one again. He mentioned that having an accordion at home would help him stay mentally engaged and alleviate the boredom he often experiences when spending long hours alone. When we asked if he would be open to us helping to source an accordion for him, he responded with great enthusiasm and happiness. Therefore, we would like to kindly request for an accordion musical instrument on his behalf, as it could greatly support his cognitive engagement, emotional well-being, and overall quality of life. A 61-year-old elderly woman living in a 2 room rental flat is on SSO ComCare assistance and unable to work due to medical reasons. M58 is single and has no next of kin. He underwent surgery to remove his voicebox due to advanced laryngeal cancer, which has rendered him permanently unable to speak. Consequently, he relies on his handphone to communicate with his SSO officer and MSW by typing. M58 recently lost his handphone, causing him significant stress as he can no longer keep track of his upcoming medical appointments. He has been permanently declared unfit for work.

Client is a 60-year-old cancer patient who is unfit for work. She has recently completed chemotherapy and will undergo daily radiotherapy. She lives alone and requires additional nutritional intake as a result of the change in her appetite and inability to process food normally.
