Client has been the main caregiver for her 12-year-old nephew since his birth, and they live with client's mother in a rental unit. Client's mother was the sole breadwinner but she had to stop working due to poor health. Client has been trying to take on ad-hoc jobs, which is barely enough to cover their basic needs. Client's mother intends to continue working once her health improves, and she has been medically compliant in the hopes that she will achieve that goal. Client is also hoping to find full-time employment once mother's health stabilises. Due to their current financial constraints, the family is seeking assistance to obtain a table console as their current one is no longer in a good condition. Client lives with husband, b/g HTN. Does not own a BP monitoring set but is keen to acquire one to start monitoring BP.

Ct is wheelchair bound, multiple secrosis and fully on comcare services. She is currently on diaper due to her medical, mobility and caregiving issues. Her 2 children are schooling and are unable to assist her financially.

We would like to seek assistance in the form of donations for a senior who is currently undergoing cancer treatment and requires ongoing nutritional and caregiving support. In addition to the challenges associated with cancer treatment, the senior has underlying dementia, resulting in increased care needs and a higher level of dependence for daily living activities. Maintaining adequate nutrition is essential to support treatment outcomes and overall well-being, while continence care products are required to ensure comfort, dignity, and proper hygiene. Although financial assistance has been secured to cover medical treatment and healthcare-related expenses, funding support does not extend to essential household consumables and nutritional supplements. The recurring costs of these items continue to place a significant strain on the household's resources and caregiving capacity. We would therefore like to appeal for donations of the following items: DiBen Milk, Vanilla Flavour (300 kcal, 15g protein) Adult Pull-Up Diapers, Size M (for night-time use) These items are essential to the senior's daily care and nutritional management. Donations will help ensure continuity of care, reduce out-of-pocket expenditure on non-medical necessities, and enable available resources to be directed towards sustaining the senior's overall health and quality of life. The family consists of a single mother with three young children, two of whom are of preschool age (3 and 4 years old). The family's only pram has broken down and due to financial constraints, the mother is unable to afford a replacement. As such, the family would greatly benefit from the provision of a double pram to assist with transporting the two preschool-aged children.
