As such, they are urgently asking for a fridge or chest freezer. They are a Muslim family and are more conservative regarding food items hence would like to either get a new fridge/chest freezer or one from a Muslim family. She is currently at a transitional care facility and her granddaughter is looking for a donor who can provide a wheelchair to support her grandmother once she returns home. Background: Madam S is a resilient single mother who is currently managing multiple chronic conditions — hypertension, Type II diabetes, and chronic kidney disease (CKD) requiring dialysis three times a week. Despite her health challenges, she continues to support her children, ensuring their daily needs are met and that they remain under stable care. Current Situation: Madam S works 2 to 4 days a week, on alternating day (9 a.m. – 9 p.m.) and night (9 p.m. – 9 a.m.) shifts, to provide for her family. Her irregular working hours and ongoing dialysis treatments significantly limit her income and physical stamina. She has no regular partner support and relies mainly on her own effort, with occasional help from friends and relatives who assist in caring for her children during her dialysis sessions and work shifts. Need: A blood glucose monitoring machine would greatly support Madam S in managing her diabetes more effectively between dialysis sessions. Currently, she does not have a reliable way to monitor her blood sugar levels at home, which puts her at higher risk for hypoglycemic or hyperglycemic episodes. Intended Outcome: With access to a blood glucose monitor and test strips, Madam S will be able to: • Track her blood sugar levels regularly and adjust her diet or medication accordingly. • Better manage her condition, reducing complications and hospital admissions. • Maintain stability for her children through improved health and fewer medical disruptions. Ct is a single mother of 3 children aged 8-15 years old, who was a victim of spousal abuse. She and children are currently in a crisis shelter, but had just secured their own rental unit. They need urgent support to have the basic household items in the rental unit, before moving in and starting anew.

Patient is single and is suffering from a leg/nerve condition, resulting in her inability to continue working. She is unemployed and relies on various government payouts in order to manage her daily living expenses. She is unable to rent out her flat for an income due to probate complications after her late mother's passing. She needs help to replace her fridge that is over a decade old.

Client is a low-income senior with health and mobility issues. He is unable to work due to his health issues, and is dependent on public financial assistance. He requires a glucouse monitoring device to track his blood sugar levels at home.
