Our client, Mdm Y is a 64 year old married woman without children. Her 80 year old husband has five children from his previous marriage. However, her husband’s children are not involved in her care, and her elderly husband is unable to provide care support too. Mdm Y has complex medical conditions. She requires supervision for her activities of daily living (ADLs) and assistance with instrumental activities of daily living (iADLs). Mdm Y requires a wheelchair to ambulate in the community and her medical appointment visits. However, her current wheelchair is malfunction and safety is compromised; but due to financial constraints, she cannot afford to purchase a new unit. A working used/new wheelchair will provide safety and reduce fall risk for her.

Client has 5 children, age 6, 5, 3, 2 and 9 months old. Client has been unemployed ever since the birth of first child and is sourcing out for full-time employment whilst husband is the sole breadwinner. Family is currently receiving financial assistance as well however family hopes to obtain a travel cot for their youngest child as a normal cot is unable to fit in their flat due to space constraint. Whilst family has obtained a cot, they hope to be able to receive a mattress in 94cm (length) x 66cm (width) as the cot they sourced for does not come with a mattress.
