Grant a Wish

I am writing on behalf of a 78-year-old male client diagnosed with mild dementia who is currently living alone. During my visits with him, it was observed that he spends most of his time at home by himself, aside from occasionally going out to nearby coffee shops for his meals. As part of our efforts to encourage him to engage in more meaningful daily activities to help slow cognitive decline, we explored possible activities that he might enjoy. During our conversation, he shared that he used to play the accordion in the past and expressed how meaningful it would be if he could have one again. He mentioned that having an accordion at home would help him stay mentally engaged and alleviate the boredom he often experiences when spending long hours alone. When we asked if he would be open to us helping to source an accordion for him, he responded with great enthusiasm and happiness. Therefore, we would like to kindly request for an accordion musical instrument on his behalf, as it could greatly support his cognitive engagement, emotional well-being, and overall quality of life. Due to client's medical condition, she needs a walker with wheels to assist with her mobility. However, client shared that she often feels tired when walking and require a foldable chair for her to bring along, so that she can sit from time-to-time.

Family has 2 school going children who requires a 3 door wardrobe for hanging the clothes. Their current wardrobe is damaged. Client is currently unfit to work and spouse is the full time care giver.

Family has expressed a need for a double-decker bed with mattresses for the mother and her son due to the current sleeping arrangements in the household. At present, the family has limited bedding and sleeping space, which may affect their quality of rest.

Single mother staying in Woodlands rental flat with 2 young children. The family has a tight budget for their daily needs and require additional support for the festivities. Client has requested for Hari Raya goodies (kueh) and clothes for her 10-year-old son and 9-year-old daughter.

Client is a big family of 12 with school going children. Finances are very tight as they have many children to provide for. Clients' children have grown out of their Hari Raya clothes and hope to receive some Hari Raya clothes to wear for the upcoming celebrations.

Ct is a single mother caring for 2 children. She has to pay rent to her parents and finds it hard to financially care for her children.

Family has 8 school-going children requires diapers (XL, tape) for their youngest child. Mom is a home maker, and father works part-time due to his health condition.
