We are reaching out on behalf of a senior who is wheelchair-bound and living alone without any next-of-kin. She is currently estranged from her family and faces significant financial challenges. Due to limited mobility and obesity, she relies on adult diapers to manage daily needs, especially during transfers to the toilet. Her current usage is 6 pieces per day, which places a heavy burden on her monthly expenses. Client is a homemaker, supporting her elderly father who suffered from a stroke, and is bedridden. She is also financially supporting her maternal grandmother with the purchase of adult diapers, without any household income. She is reliant on her savings and ad-hoc earnings from her home based baking business. Client is also a caregiver to her school going daughter. Client requires support in purchasing milk powder for her elderly father in sustaining his feeding needs.

Client is an elderly who is on LTVP and requires diapers support as they faces financial difficulties due to high medical costs and needs. Son is the sole breadwinner of the family and is supporting elderly with her needs. Family would benefit from the diapers support to alleviate their financial strain

Background: Madam S is a resilient single mother who is currently managing multiple chronic conditions — hypertension, Type II diabetes, and chronic kidney disease (CKD) requiring dialysis three times a week. Despite her health challenges, she continues to support her children, ensuring their daily needs are met and that they remain under stable care. Current Situation: Madam S works 2 to 4 days a week, on alternating day (9 a.m. – 9 p.m.) and night (9 p.m. – 9 a.m.) shifts, to provide for her family. Her irregular working hours and ongoing dialysis treatments significantly limit her income and physical stamina. She has no regular partner support and relies mainly on her own effort, with occasional help from friends and relatives who assist in caring for her children during her dialysis sessions and work shifts. Need: A blood glucose monitoring machine would greatly support Madam S in managing her diabetes more effectively between dialysis sessions. Currently, she does not have a reliable way to monitor her blood sugar levels at home, which puts her at higher risk for hypoglycemic or hyperglycemic episodes. Intended Outcome: With access to a blood glucose monitor and test strips, Madam S will be able to: • Track her blood sugar levels regularly and adjust her diet or medication accordingly. • Better manage her condition, reducing complications and hospital admissions. • Maintain stability for her children through improved health and fewer medical disruptions.
