Home hospice patient with increasing shortness of breath requires portable oxygen concentrator for her trips to the hospital.

Family has an elderly mother who requires the usage of diapers frequently every day. She requires 10 pieces per day given her physical condition and is being supported by her daughter and son-in-law.

Client is a 60-year-old cancer patient who is unfit for work. She has recently completed chemotherapy and will undergo daily radiotherapy. She lives alone and requires additional nutritional intake as a result of the change in her appetite and inability to process food normally.

Client is a middle age lady caring for her aged mother who is currently bed-bounded. Given her caregiving responsibilities, she is unable to work and has limited capacity for home-based employment given her personal ailment as well. Family requires regular adult diapers (size L, tape), underpads, wet wipes and other associated assistance as client is a full time caregiver with nil income. The husband has recently been diagnosed with diabetes and requires regular monitoring of his blood sugar levels. However, the family is unable to afford a blood glucose meter due to their current financial situation. Having this device will help him manage his condition safely, prevent complications, and reduce the need for costly medical visits. The client often feels physically weak, making it difficult for her to secure stable employment. She is known to SSO and currently receives financial assistance. While she has adequate informal support, the client prefers not to trouble her support network and will only seek help when she has insufficient funds. She stays alone in a rental flat.
